Contract Duration and Automatic Renewal: THIS CONTRACT REMAINS IN FORCE UNTIL THE CLIENT SUBMITS THE OFFICIAL CANCELLATION FORM.
CANCELLATION REQUESTS MUST BE SUBMITTED AT LEAST 30 DAYS PRIOR TO THE NEXT BILLING DATE.
Members enrolled in autopay may freeze their membership once per year. To do so, a completed freeze request form must be submitted in writing at least 30 days before the next billing date. Since all billing occurs on the 1st of each month, the freeze will take effect at the start of the next billing cycle.
During the freeze period, a fee of $15 per month will be charged. Memberships may be frozen in one-month increments for a maximum of six months. While on an active freeze, members may attend classes at the drop-in rate.
(a) I agree to purchase the PAR/Seniors Membership for $70 per month, charged automatically to my credit card or debited from my checking account on the 1st of each month.
(b) I certify that I am the holder of the credit card or an authorized signer on the checking account provided.
(c)I understand that the official cancellation form must be submitted at least 30 days before the next billing cycle. No prorations or refunds will be issued for cancellation requests submitted after the 1st of the month.
(d) If my payment fails, I will be notified and a $10 late fee will apply if valid payment information is not provided within 10 calendar days.
(e) I understand that my membership services may be deactivated if my account is more than 30 days past due. This does not stop monthly fees from accruing unless the official cancellation form is submitted.
(f) I have read and acknowledge the freeze policy and agree to its terms as written above.
(g) I understand that delinquent balances may be sent to collections, and I am responsible for any associated fees.
